This application form will not be received until you click the Submit button on the last page at which point you will then be redirected to process your application fee online.

 

Once your completed application form with essays, application fee, photograph, and copy of 200 hour certificate (if applicable) have been received, we reach out regarding your acceptance. Please allow 2-3 business days to review your submission. We look forward to being in touch!
